ID is a 70 year old gentleman who sustained a stroke in November 2023.
He was admitted to Laverstock Care Centre for a period of 26 weeks of inpatient rehabilitation funded by Wiltshire ICB. A further 12 weeks of funding was agreed with the aim of getting him home to his property that was not ideal environmentally.
• Left sided weakness
• Left sided quadrantinopia
• Overactivity of right side
• Anxiety and anticipation of pain response
• Cognitive impairment including reduced attention and reduced insight into impairments
• Cognitive communication disorder
• Vestibular dysfunction
• Needed supervision when eating
• Difficult bed mobility as he was a pusher with anxiety, thus hindering the movement
• Full hoist for all transfers
• Reduced sitting balance with reduced awareness of midline
• Dependent for all personal activities of daily living including toileting, washing, and
dressing. These tasks took a significant amount of time as he needed lots of reassurance
• Full multi-disciplinary team input including assessment and treatment progression with physiotherapy, occupational therapy, speech and language therapy, psychology, rehab Assistant and rehab Consultant.
• Neurotechnology including the FES bike, Exomotus, Omego bike, M2 robot and the ARJO electric standaid,
• Implementation of a sleep system
• Orthotics review
• Spasticity review and administration of botox
• Comprehensive discharge planning to enable him to get home to a property that was not ideal.
• Improved ability to participate in rolling, requiring assistance of 1.
• Improved midline awareness and sitting balance.
• Able to transfer with molift stand aid with assistance of 2 carers.
• Has practiced standing transfer with molift with partner as an assistance of 1 with the aim to
do this when at home when the carers were not available.
• Has mobilised 8 metres with facilitation with an ARJO Encore standing aid with assistance of 2.
• Can sit out in a standard commode for a shower everyday.
• Able to participate in elements of personal care.
