AS | Stroke | Intensive Neurotherapy Centre

AS is a 69 year old man who has been attending Hobbs Rehabilitation since 2019 after he had a left ischaemic infarct stroke. While being with Hobbs he has attended Physiotherapy, Occupational Therapy, and Speech and Language Therapy.

Main Areas Affected

• Arm and leg weakness
• Spasticity in his arm and leg
• Expressive aphasia

Functional Ability on Admission

• Used a wheelchair for moving around
• Able to walk short distances with help and a zimmer frame
• Carer support for all daily activities
• Able to say yes/no

Hobbs Rehabilitation Input

• He received PT/OT/SLT as an outpatient
• Stretching and strengthening of his arm
• Spasticity management – onwards referral for botoxium injection – splint for his elbow and custom made splint for his wrist
• Helping him to use his right hand as a stabiliser for daily activities
• Targeted strengthening for his leg
• Gait re-education and standing work including balance
• Use of Andago V2.0, Tymo, Myro, Lexo, C-mill VR, Omego, NeuroVirt, Diego, Amadeo, ArmeoSpring Pro
• FES – he uses a programme for the subluxation of his shoulder
• Working alongside orthotics to find appropriate ankle splint
• Aiding with returning to sport – Tennis

Functional Ability on Discharge

Ongoing physiotherapy but:
• Walking independently with a hiking pole
• Able to complete all daily activities independently, with use of his right arm to stabilise
• Able to communicate what he wants to say with time and strategies
• Incorporating rehabilitation appropriate into daily life

Outcomes

Goals

To be able to feel more confident in mobility
Has progressed from using a wheeled zimmer frame and a quad stick to walking independently. Able to walk outdoors with a hiking pole. Able to mobilise to allow to participate in tennis within the clinic
To be able to manage his spasticity
Has a splinting routine, attends clinic that hobbs link with that provides botulinum injections, stretching routine, and strengthening routine.
To attend horse racing competitions to see his horses race
Attends horse races with the support of friends and carers – he feels more confidence in his walking and speech
