Following assessment, our team will consider your individual needs, goals and circumstances to determine the most appropriate rehabilitation setting and level of support. Your rehabilitation may move between settings as your needs change, with inpatient, home-based and outpatient rehabilitation available where appropriate. Access to our multidisciplinary team can also be provided where input from different specialists would be beneficial.
